Chronic Diseases Prevention


Many chronic diseases are preventable, or react more favorably in terms of management and medical treatment, when healthy behaviours, such as controlling body weight, eating nutritious foods, avoiding tobacco use, and increasing physical activity are adopted.

The Chronic disease management and Preventive health program supports GPs, allied health professionals, practice nurses and practice staff to implement and optimise preventive health strategies by providing information, resources and assistance via practice visits, telephone/email and workshops.

external link Get Healthy Information and Coaching service: A free, confidential telephone and coaching service that helps people make lifestyle changes. 

external link Go4Fun: A free healthy living program for kids aged 7 – 13 who are above a healthy weight.

external link GP Exercise Referral Scheme: The program runs in Sutherland Shire and St George regions which support to help people become more active with a customised exercise plan provided by Accredited Exercise Physiologist.

external link Heartmoves: A gentle physical activity program suitable for anyone who hasn’t done any exercise in a while.

Allied Health Professionals

Allied Health Professionals can provide healthcare services relating to the identification and prevention of chronic health diseases. They can assist GPs and Practice Nurses to implement and optimise preventive health strategies for improved patient outcomes.
