About the Scheme

The GP Exercise Referral Scheme (GPERS) is specifically designed for people referred by a GP or health professional to prevent or manage certain medical conditions.

The GPERS program includes a customised exercise plan and support to help you become more active and improve confidence to make positive lifestyle changes. An ESSA Accredited Exercise Physiologist will conduct an initial and final assessment and provide a tailored exercise prescription. The program is designed to meet your needs and participation will be supervised by a qualified fitness professional.

Get moving! Stay well!

The program aims to improve your confidence with lifestyle change and motivates you to increase your physical activity levels. Get moving! Stay well is designed to help you prevent long term health complications and take positive action if you suffer from:

  • Diabetes
  • Pre-diabetes (IFG/ IGT)
  • At risk of developing diabetes
  • Overweight (BMI greater than 30)
  • Waist circumference > 94cm (male)
  • Waist circumference > 80cm (female)
  • Hypertension
  • Raised cholesterol levels
  • Osteoporosis
  • Osteoarthritis (mild-moderate) 
  • Polycystic Ovarian Syndrome

Community Maintenance

This program offers the option of community exercise for people that have completed cardiac or respiratory rehabilitation at St George or Sutherland Hospital.

It is important to consult your GP or Specialist to ensure that the program is appropriate and that you meet the referral criteria.
