HealthPathways Sydney is an online local health information portal to support local GPs and local clinicians while they are seeing their patients. It provides information on how to assess and manage medical conditions, and how to refer patients to local specialists and services in the most-timely way and is accessible to General practitioners and Allied Health professionals from 31 March 2014. The name HealthPathways reflects the referral lines or 'pathways' which link patients to the best treatment, local service or specialist. HealthPathways is aimed at General Practitioners, but can also be used by hospital specialists, practice nurses/managers, Residential Aged Care Facilities and community allied health providers. HealthPathways is a dynamic collaboration between Central and Eastern Sydney PHN and Sydney Local Health District. General Practitioners, hospital specialists and community and allied health providers are all involved in creating HealthPathways. HealthPathways is based on a highly successful model of collaboration developed in New Zealand. Over time it has resulted in significant improvements in the way hospital and General Practice share the care and clinical management of patients. It has reduced costs and improved the quality of care to patients.
How the Program operates
HealthPathways are developed by local GPs, Clinicians from Sydney Local Health District and private allied health providers and clinicians. The HealthPathways Sydney Program Team convenes work group meetings with primary and secondary clinicians to identify what is working well and areas for improvement. Based on discussion and shared decision making, local clinicians determine local solutions (Pathways) for implementation. These pathways will provide information on assessment of symptoms and outline management options including access information about local services and referral process to services and specialists. HealthPathways Sydney Program Team facilitates this development process. The pathways then go through a process of review and formatting ready for publication on the HealthPathways Sydney website.

Access to the HealthPathways Sydney live site is restricted to local General Practitioners, Allied Health Professionals and Sydney Local Health District personnel.
