The South Eastern Sydney Local Health District and the Central and Eastern Sydney PHN are inviting General Practitioners practising in the St George and Sutherland regions, who also have a Practice Nurse, to participate in an innovative program developing a model of care for the diagnosis, treatment and management of patients with chronic heart failure.

The program offers specialist clinical support to develop greater confidence and skills in the assessment and management of patients with chronic heart failure in the primary care setting.

The program is currently recruiting interested GPs with Practice Nurses to until March 2017.

The table (developed initially by the South-East Qld Refugee Health Clinical Advisory Group) aims to provide a quick-reference guide to general practices who currently provide or are considering providing health care services to people from a refugee background. It is by no means an exhaustive list, but aims to serve as a tool to support health care for this vulnerable group and the development of appropriate primary care services.
