Be a GP Representative on the Heart Failure Integrated Care Project Steering Committee - Expression of Interest

The Heart Failure Integrated Care Project in the St George and Sutherland regions is providing great resources and clinical support to assist general practices who manage patients with heart failure. The project will be run by CESPHN and SESLHD until June 2017.

Be part of this innovative chronic disease integrated care model!

We need a GP representative to strengthen the project’s great relationship between primary and secondary health care settings. The chosen GP needs to employ a practice nurse.

Meetings are held on the second Tuesday of each month from 7.30 am - 8.30 am at St George Hospital. After attending the initial meeting in person, you can attend by teleconference. This early meeting time means you won’t miss your full practice day.

A small payment will be provided for your attendance and pre-reading time.
