Strengthening Care for Children project: An exciting opportunity for your practice to participate in a 12-month project

23 February 2021

An exciting opportunity for your practice to participate in a 12-month project with the Sydney Children’s Hospital (SCH), Randwick and Central and Eastern Sydney Primary Health Network (CESPHN).

The Strengthening Care for Children (SC4C) project is an expansion of a project that was successfully piloted with general practices in Victoria (north west Melbourne) between 2017–19. You can watch a brief video and read our case study for more information on the project.

The SC4C project is testing a new model of care that aims to increase GP confidence and knowledge in paediatric care and reduce paediatric referrals to hospital emergency departments and outpatient clinics. The model seeks to do this through the development of an educational relationship between participating GPs and a paediatrician from the SCH.

We are looking for 11 practices in the central and eastern Sydney region to participate. The SC4C project involves a SCH paediatrician leading monthly in-practice case study discussions and providing specialist paediatric phone and email support to GPs. Paediatricians will also conduct co-consulting sessions with participating GPs in your practice to provide shared care to your paediatric patients.

Practices in the trial will receive:

  • $7,000 total payment made in instalments when completing project milestones
  • Co-consulting sessions in your practice between a GP and a SCH paediatrician (weekly to fortnightly)
  • Paediatric support (via email or phone) from SCH paediatricians
  • Monthly 60-minute case study discussions at your practice with SCH paediatrician
  • Education and training in paediatric care
  • RACGP CPD points for GPs who participate in case study discussions and co-consultations
  • Project support by regular practice visits from the CESPHN and SCH project team

Eligibility criteria

To be eligible for the trial, practices must:

  • Be one of the general practices pre-identified by the SCH
  • Be an accredited general practice located within the CESPHN region
  • Have Best Practice or Medical Director 3 software
  • Be able to complete the project over a 12-month duration between May 2021 – April 2023
  • Have the infrastructure to facilitate co-consulting and manage paediatrician bookings
