New mental health referral pathways

The South Eastern Sydney (SES) HealthPathways team is pleased to announce 7 new Mental Health referral pathways went live during September – right on time for Mental Health Month and World Mental Health Day! This brings the total number of Mental Health referral pathways up to 20 and 6 Mental Health clinical pathways.

These pathways include service referral options to help Mental Health consumers recover from, manage or prevent such conditions as depression, anxiety, PTSD, addiction, psychosis, mood disorders, personality disorders, social isolation, suicide ideation, and grief and loss. The pathways also make it easier for GPs and primary care to find the right services for their patients and improves the quality of referrals hospitals receive from them. There are still many more Mental Health clinical and referral pathways that will go live over the next few months!

The SES HealthPathways team would like to provide special thanks to each of the GPs, Mental Health Clinicians, Managers, Administration staff and services across the partner organisations who provided their input and support to get these pathways up.

We look forward to the positive impact these pathways will have on Mental Health consumers and our health system.

HealthPathways is an online tool for GPs (and other Healthcare workers) used during patient consultations. It contains locally agreed and up to date information on how to assess and manage patients, and how to refer patients to local services. SES HealthPathways is a partnership between CESPHN, South Eastern Sydney Local Health District (SESLHD), St Vincent’s Hospital Network (SVHN) and Sydney Children’s Hospital Network (SCHN).
