RACF Outreach Model of Care

The RACF Outreach Model of Care was implemented in February 2018 with the goal of maximising patient centred care and quality of life by providing a proactive, comprehensive, holistic, integrated and specialised aged care service for the over 4,500 residents of aged care facilities across the District, within their usual accommodation. The service aims to provide ‘the right care, at the right time, in the right place.’ We currently have over 200 residents registered to the service, and are keen to improve our communication and relationships with local GPs.

Typical referrals to our service include requests for Comprehensive Geriatric Assessment, EOL care, behaviour management, delirium, falls and wound care. Referrals are triaged for urgency at our daily team meeting.

We are able to work in liaison with our colleagues in SLHD including palliative care, continence and HITH services and are expanding our capacity with Telehealth to improve access to specialist services such as dermatology. The service also offers targeted education for facilities to increase staff knowledge and capability in key areas including wound care, palliative care, behaviour and falls.

Access Care Team (ACT)

7 days a week 8:00am - 8:00pm Including Public Holidays

ACT is the single point of telephone contact to support and provide advice to staff, General Practitioners (GPs) and health providers in the clinical care of residents of RACF across Sydney LHD.

Recent inclusions include a notification service from our ACT service, so that if one of your patients is referred you receive notification by fax that day, and an extension of our direct clinical service till 8pm on weekdays.

We look forward to strengthening partnerships and consolidating positive working relationships with RACF staff, GPs, PHN and NSW Ambulance.
