SLHD Community Paediatrics now accepting HealthLink e-Referrals

Sydney Local Health District is pleased to announce that from Wednesday 11 September 2019, e-Referrals can be made to Community Paediatricians using the HealthLink SmartForm system.

HealthLink enabled general practices can refer patients aged 0-16 who need assessment and management for development and behavioural concerns to services provided at Community Health Centres within Sydney Local Health District.

e-Referrals are now being accepted for osteoporosis and bone mineral density scan requests at Concord and Royal Prince Alfred Hospital.

Services now accepting HealthLink e-Referrals:

  • Community Health Centres - Community Paediatrics
  • Concord Hospital - Osteoporosis and BMD service
  • Concord Hospital - Haematology service
  • Royal Prince Alfred Hospital - Osteoporosis and BMD service
  • Royal Prince Alfred Hospital - Haematology service
  • Royal Prince Alfred Hospital - Rheumatology service
