EOI: Person Centred Medical Neighbourhood Program - GP Clinical Leads

The Person Centred Medical Neighbourhood Program (PCMNP) has played a vital role in supporting Practices’ Readiness for Change. By basing the program around the first four of Bodenheimer’s 10 Building Blocks of High Performing Care: engaged leadership, data driven improvements, patient registration and team base care, we have successfully developed a structured approach which has been easy to follow and accessible to all levels within the general practice setting. It has certainly given general practice valuable insight and methodology into how to embed a “Person Centred Model of Care” which in return has provided excellent outcomes to those practices that are participating.

Central and Eastern Sydney PHN is seeking expressions of interest for GP Clinical Leads to join the established reference group - PCMN GP Clinical Leads. This reference group provides overarching clinical advice to guide the development and implementation of the PCMNP.

The PCMN GP Clinical leads will be selected based on the following key attributes:

  • Registered general practitioner with a minimum of five years experience at a general practice within the Central and Eastern Sydney PHN region and a deep understanding of our local issues
  • Ability to build rapport with other GPs and supporting their general practice’s effort to transform into a PCMN model with mentoring and guidance;
  • Desirable, current or previous experience as a general practice principal

The PCMN GP Clinical leads will meet four times per year and you will be remunerated for your time.
