New project with Kirketon Road Centre and St Vincent's Hospital

Central and Eastern Sydney PHN is pleased to announce new initiatives in the form of its Inner City Outreach Project and After Hours Primary Care and Care Coordination to Vulnerable Populations.

Kirketon Road Centre will operate a mobile outreach team with an equipped van to regular locations in central Sydney. The service will provide primary care, care coordination and linking to health services.

Furthermore, St Vincent’s Hospital will run clinics within Wesley Mission, Wayside Chapel, Foster House and Teirney House. The service will also provide mobile outreach on weekends, telehealth support and care coordination.

The project is aimed at vulnerable populations, including people with a history of injecting drug use, “at risk” young people (people under 25 years with a street based lifestyle in the inner city), Aboriginal and/or Torres Straight Islander people in contact with KRC, sex workers, people who may be experiencing of at risk of homelessness and those who present to the emergency department but do not wait for treatment.

This project aligns to the needs assessment where it states Central and Eastern Sydney PHN will continue to commission services to target frequent users on after-hours services with homeless residents being identified as one of these populations.
