Uniting War Memorial Hospital Geriatric Flying Squad

With winter and the flu season now upon us, the War Memorial Hospital Geriatric Flying Squad (GFS) would like to take this opportunity to advise local health care providers and general practitioners of our services and of how to make a referral.

The GFS is a rapid response multi-disciplinary team who provide comprehensive geriatric assessment and short term case coordination for older people living in the community or a residential aged care facility (RACF) and who are experiencing a subacute physical, functional or cognitive decline. The primary goal of this service is to improve access to quality care for older people in their place of residence, thereby eliminating avoidable transfer to hospital emergency departments.

Additionally, the GFS offers an after-hours service Monday-Friday for residents of RACFs. The after-hours service is staffed by a Nurse Practitioner or Transitional Nurse Practitioner (NP/TNP) and operates Monday-Friday 1 pm to 8.30 pm and on Saturday and Sunday, 8 am to 7.30 pm.

The after-hours GFS covers RACFs in the Prince of Wales Hospital (POWH), War Memorial Hospital (WMH) and St Vincent’s Hospital (SVH) catchment areas in consultation with on-call geriatricians. It offers rapid response assessment and management of a number of health concerns including new onset confusion (delirium), fever, urinary tract infections, constipation, cellulitis, flu, community acquired pneumonia, post falls review, palliation/end of life care and non-acute cardiac issues such as CCF. Services offered include prescribing and commencement of intravenous antibiotics and diuretics, oral antibiotics and other pharmacotherapeutics, sub-cutaneous rehydration, ECG, post-void bladder scanning, non-routine IDC insertions.
