Make a nomination for the Community Wellness Award!

 

The Sydney Health Community Network invites your community organisation to nominate for the Community Wellness Award. The Award aims to recognise the activities of agencies or individuals who strive to improve the health and wellbeing of vulnerable or disadvantaged people who live and/or work in the Central and Eastern Sydney Primary Health Network (CESPHN) region.

We hope that you will take the time to tell your community’s stories to general practitioners, allied health providers and other community organisations. This is your opportunity to show the benefit of early intervention and the impact that increased community agency and connection has on wellbeing.

There are three Award Criteria for Community Wellness:

  1. Lived experience has been integrated and valued in service design and delivery to promote community inclusion, improved health literacy and personal wellbeing
  2. Activities to improve health literacy have enabled early intervention and/or prevention
  3. Person centred services have enabled access to relevant support and resulted in improved perception of wellbeing.

Projects and services that demonstrate strength in any two of the above criteria are encouraged to self nominate. Recognition in the Awards will increase the opportunity for grant monies, partnerships and further community engagement and wellbeing.

Award nominations close on 31 July and the award event will be held on 24 October.
