Southcare Occupational Therapy Service

Who we are: We are a team of experienced Occupational Therapists based at the Sutherland Hospital in the Southcare Building. Southcare is the Aged and Extended Care Service of the Sutherland Hospital.

What we do: We assess clients in their own home and community with the aim of improving their safety and independence. We provide falls assessment and education, we recommend specific home modifications and equipment to support safety and independence within the home and to support the carer. We provide assessment for equipment to prevent pressure injury and to promote healing of existing pressure injuries. We make referrals to other health professionals and work as part of a team to help achieve the best health outcomes for your patients.
NB: We also assess and prescribe equipment for community access eg scooter, but require medical clearance first from the patients GP if applying to Enable NSW for prescription of this equipment.

Who we see: We currently provide a free service to residents of the Sutherland Shire who are either aged or disabled and living in the community, we see adults over 18 years of age. We don’t see DVA Gold card holders or those who reside permanently in a nursing home.

How to refer to us: For those who are under 65 years old and do not have an active NDIS plan - refer by emailing or faxing the Southcare Access and Referral form to Southcare intake.

For those over 65 years old - refer via My Aged Care, stating on the referral that the referral is for Southcare Occupational Therapy.
