RPA Women and Babies Referral Forms

RPA Women and Babies have updated both their pdf Obstetric Referral Form (336 KB) and pdf Gynecology Referral Form (333 KB) . Updated templates for importing into your clinical software will be available shortly.

PLEASE NOTE : Appropriate triage and management can only be arrange if referrals include adequate clinical information. Some referrals received from GPs have been illegible, contain minimal clinical information and omit important details eg EDB. It is very time consuming for hospital staff to follow-up may cause delay is appropriate referrals.

CESPHN Practice Support Staff are also informed when new or updated referral forms are available so they can inform practice staff at practice visits.

Introduction of Reproductive Medicine Clinic - RPA Women and Babies

The Department of Reproductive Endocrinology & Infertility (DREI) will be modifying its service and introducing a Reproductive Medicine clinic from May 2018.

Where: Women & Babies Ambulatory Care, Level 1

When: Friday pm - From 4 May 2018

The aim of this clinic is to provide a tertiary consultation service for:

  1. Disorders of sexual differentiation (transition from Paediatric services i.e. Sydney and Westmead Children’s hospitals);
  2. Pubertal disorders (primary or transition from Paediatrics) and adolescent gynaecology (age ≥ 16yrs);
  3. Polycystic ovary syndrome;
  4. Hirsuitism and virilization;
  5. Amenorrhoea and oligomenorrhoea;
  6. Other menstrual disorders;
  7. Female and Male Oncofertility.

Clinic Referral – FAX completed document referral form (98 KB) to DREI 9515 7976. An appointment for the patient will be organised and the patient informed.
