Request for Proposal: Diabetes education in pregnancy for migrant communities

Central and Eastern Sydney PHN is requesting proposals for the provision of community-based educational programs to improve knowledge and promote client management of the risk factors associated with diabetes in pregnancy to women of child-bearing age (and the community) from specific CALD communities.

Services will be commissioned in line with Department of Health guidance and will ensure that the service is:

  • evidence-based and person-centered with optimal use of resources, including workforce and infrastructure
  • focussed on the consumer and is culturally appropriate.

The project will target those CALD communities with the highest incidence of women diagnosed with diabetes in pregnancy in the SLHD area. It is expected that key areas of the project will include the delivery of community-based educational programs to the target audience and engaging with target CALD communities and stakeholders and develop or source "in language" resources.

CESPHN invites suitable allied health professionals and/ or organisations to submit a request for proposal.
