EOI for Staying Well at Home Steering Committee

Expressions of Interest are being sought for membership to the Staying Well at Home Steering Committee.

Interested in the keeping older patients in your local community well and out of hospital during winter?

GPs and practice nurses located in the Sutherland and Canterbury local government areas are invited to apply for membership to the Steering Committee (members will be remunerated for their time).

The Staying Well at Home initiative aims to pilot innovative partnerships and models of care that enable people aged 75 years and over with CCF, COPD, or frailty to better self-manage their chronic condition and avoid hospitalisation.
